We started with one problem and found it at every level.
Why we started
"I spent some years in trauma intensive care watching patients come back for problems that were preventable between visits."
We started Premier Care Coordination to solve one problem: people don't know how to navigate a fragmented healthcare and community system. As we worked with healthcare organizations and talked to communities, we found that problem at every level, for seniors and families, for physician practices and hospitals, and for cities and communities.
Where we're going
Every community gets a front door, and no one falls through the gap between social services and healthcare. No one should have to navigate it alone.
