What is Premier Care Coordination?

Who do you work with?

Physician practices, federally qualified health centers (FQHCs), rural health clinics (RHCs), skilled nursing facilities, assisted living facilities, hospice organizations, and specialty groups. If you have Medicare patients and a billing practitioner, we can likely partner with you. We will confirm fit in the discovery call.

Does Premier Care Coordination replace our existing staff?

No. We operate entirely outside your organization. Your staff does nothing differently. The billing practitioner identifies eligible patients during a visit, and from that point forward we handle all monthly coordination of care, documentation, and reporting. Your team receives a clean monthly summary ready for submission.

Do we need to change our EHR or learn new software?

No. Premier operates on a proprietary platform purpose-built for concurrent CHI, PIN, and CCM delivery. There is no platform license for you to manage, no implementation lift for your IT team, and no training requirement for your staff.

How quickly can we get started?

After a discovery call confirms fit, onboarding typically moves quickly. The pace from there depends on your panel size, identification workflow, and how soon eligible patients are visiting. We will give you a realistic timeline once we understand your situation.

What does the discovery call actually cover?

A short conversation about your patient panel, which programs you are or are not currently running, what your goals are, and how a partnership would work mechanically. No pressure, no pitch deck. If the fit is clear, we move to next steps. If it is not, we will tell you.

What is Community Health Integration (CHI)?

A Medicare program introduced January 1, 2024 that reimburses for addressing the social determinants of health that prevent patients from following their care plans. Transportation, housing, food insecurity, and social isolation all qualify when documented as affecting treatment. There is no minimum chronic condition requirement.

What is Principal Illness Navigation (PIN)?

A Medicare program reimbursing for structured patient navigation for Medicare patients with serious high-risk conditions, including cancer, congestive heart failure, COPD, dementia, HIV/AIDS, severe mental illness, and substance use disorder. We operate as nurse-led care coordinators for PIN delivery, which is a higher standard than the community health worker baseline CMS designed the program around.

Can CHI, PIN, and CCM all be billed for the same patient in the same month?

Yes. CMS explicitly permits concurrent billing when time and effort for each program are documented separately and not double-counted. The documentation discipline this requires is exactly what Premier was built to deliver. We will walk you through what this looks like for your patient panel in the discovery call.

Why does it matter that your team is nurse-led care coordinators?

CMS allows community health workers to deliver CHI and PIN, and most contracted vendors use CHWs because they are less expensive. We operate as nurse-led care coordinators for these services. For the practitioner supervising under their NPI, our nurse-led approach with clinical judgment, medication awareness, escalation recognition, and documentation quality meaningfully reduces liability exposure and improves patient outcomes.

Do you work directly with individuals or families?

On a limited basis, yes. Our primary focus is contracted delivery for practices and facilities, but we accept a small number of private engagements with individuals and families navigating complex care situations.
